LA Acting Academy, LLC located at 12840 N 19th Ave, Phoenix, AZ 85029 is great fun but it can also be physically and mentally demanding. Students need to have self-discipline in their approach to learning, actively participating, and listening to the tutor/coach and other students when required. This allows all students to have a positive learning experience and become valued members of our Academy ensemble. If a student is repeatedly disruptive in class to the point where other students are being affected, the tutor/coach will discuss with the student/parent/guardian if the student wishes to continue attending classes. If the disruptive behavior continues, enrolment will be cancelled and no refund will apply.

I certify that the above named enrollee has no condition that prohibits full participation in classes at LA Acting Academy, LLC. I release LA Academy, LLC located at 12840 N 19th Ave Phoenix, AZ 85029 and its employees, volunteers, and affiliates from all liability in connection to any personal injury and/or damage to or loss of personal property while engaged in class activity or walking/driving to/from class. In the event of a medical emergency, I grant the staff permission to engage in first aid (if trained) and if necessary to transport or have my child transported to the nearest Emergency Medical facility.
My child agrees to exercise care in the facility, and I agree to reimburse LA Acting Academy, LLC located at 12840 N 19th Ave ,Phoenix AZ 85029 for any personal breakage, damage or loss of property due to his/her negligence. It is my responsibility to cover any fees resulting from a check which has been returned by the bank. My money will be refunded only in the event that the class is cancelled.

Here’s How Payments Work:
You authorize regularly scheduled charges to your debit or credit card. You will be charged the amount indicated below each billing period. A receipt for each payment will be emailed to you and the charge will appear on your bank statement as an “ACH Debit.” You agree that no prior-notification will be provided unless the date or amount changes, in which case you will receive notice from us at least 10 days prior to the payment being collected. I understand that this authorization will remain in effect until I cancel it in writing, and I agree to notify LA ACTING ACADEMY in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. If the above noted payment dates fall on a weekend or holiday, I understand that the payments may be executed on the next business day. For ACH debits to my checking/savings account, I understand that because these are electronic transactions, these funds may be withdrawn from my account as soon as the above noted periodic transaction dates. In the case of an ACH Transaction being rejected for Non Sufficient Funds (NSF) I understand that LA ACTING ACADEMY may at its discretion attempt to process the charge again within 30 days, and agree to an additional $35.00 charge for each attempt returned NSF which will be initiated as a separate transaction from the authorized recurring payment. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law. I certify that I am an authorized user of this credit card/bank account and will not dispute these scheduled transactions with my bank or credit card Company; so long as the transactions correspond to the terms indicated in this authorization form.

LA ACTING Studio is a one-of-a-kind acting studio that brings Hollywood to Phoenix, AZ. Classes are offered year round for Kids to young Adults. Students are guided through the entire process of Acting for the Camera and Breaking into the Biz. Our student stars can be seen in many local, regional, and national Commercials, TV and Film. Class size is limited to 12 students to get maximum instruction.

Tuition: Tuition is due by the 1st of each month. All accounts are set up on automatic draft with the credit card provided during registration. By signing up at LA Acting Academy you give LAAA permission to bill your credit card for all tuition, fees and balances. Tuition will come out on the 1st of every month unless it falls on a holiday/weekend, in which case we will bill on the workday before. Cancellation of your automatic draft requires a written, 30-day notice. All tuition and fees are non- refundable and non-transferable. A $10 late fee will be applied to account balances not paid in full by the 10th. There will be a $25.00 returned check fee for any checks returned by the bank. This must be paid before the next class can be taken. Prorated classes: Prorated classes are only available for a student who starts in the middle of the month. Tuition is based on the total number of days in the year, not number of weeks. Therefore, tuition remains the same for all months. Attendance Policy: You must call the office at 623-986-5731 if you are going to be absent or late for any reason. An absence is considered excused only if it is for an audition, rehearsal or shoot related to the business. You are entitled to make up the class in the following four- week session, subject to space availability. Please coordinate this with the office. Any absence that is NOT related to the business is unexcused and cannot be made up. We understand that illness and emergencies happen and we will do our best to keep you caught up with the rest of the class should you be absent. Please remember that we cannot reimburse you for classes missed. You have made a valuable investment in yourself and you need to be present in order to get as much out of it as you can.
Resume Requirement: We believe that students gain the most when they consistently attend class. If a student wants to use LAAAas a credit on their resume they MUST attend a LA ACTING ACADEMY STUDIO POLICIES: LA ACTING ACADEMY is a one-of-a-kind acting studio that offers best training and opportunity for genuine exposure to leaders in the entertainment industry. Students Street, Phoenix AZ 85032 (Suite 162) class. If a student wants to use LAAA as a credit on their resume they MUST attend a Initial:_________ minimum of 3 consecutive months of class.
Class Cancellation: A 30-day written notice is required in order to cancel or change your membership. Drop forms must be completed in person and submitted to the Front Desk of LA ACTING ACADEMY to begin the drop period. Students are responsible for all tuition fees during the 30-day drop period. Tuition charges are based on the date of the notice, not the last class attended. Since LAAA maintains small class sizes we cannot hold a spot for a month missed or skipped. All tuition is non- refundable and non-transferable. If accounts are not paid by the 10th of each month and written notice is not given, we will charge your credit card on file the balance due. No studio fees will be refunded when cancelling.
Harassment Policy: LA ACTING ACADEMY will not tolerate harassment, bullying or other inappropriate behavior of any kind between or among its Students and/or Parents. Any Student or parent who experiences this type of inappropriate behavior should immediately contact Maja at [email protected]
Parent Observation: Parents are required to remain outside the classroom to avoid possible distraction. From time to time parents may be invited into the classroom at the end of class to see what their child has worked on and we will have a designated week where parents can observe an entire lesson.
LAAA Workshops: LA Acting Academy will continue to periodically offer workshops with the BEST Industry Professionals to further increase the knowledge of our students. As you know, the competition in TV and FILM is FIERCE and we will make sure our students have the best tools when it comes to BOOKING roles. Workshops are based on availability and Full Time LAAA students will have priority registration for these events.
LAAA Famous Showcase: “LAAA STARS” is our End of the Year Showcase where we invite agents, managers and casting directors to watch our students perform. This event always happens on one of the Saturdays in June. This event is very popular and prestigious and therefore it is very important that our actors are ready to perform. Students will have the opportunity to attend a 5-day intensive training camp where they will work with the best professionals in the industry prior to the event. Tuition for this event will be due by May 1st . Showcase eligibility and participation is at the sole discretion of LAAA staff and management.
LAAA Calendar: LAAA will observe all major US Holidays. If a holiday falls on a Saturday or Sunday, the school will be closed and classes for that week will be cancelled. LA Acting Academy's yearly schedule is as follows: Weekly classes are offered January 1st through the 3rd Saturday in June. The LAAA STARS Showcase training will be held during the one week Initial:________ of June with the actual Showcase following that end week Saturday. The school will be closed during the month of July for summer break. Classes resume August 1st through December 30th. The school will be closed during Christmas and New Year Holiday.
